To better understand the issues surrounding deaths and mortality reviews.
This is a collaborative piece of improvement work between Healthcare Improvement Scotland, Scottish Government, Public Health and Intelligence (part of NHS National Services Scotland) and the Scottish Academy of Royal Colleges. The work will design, test and implement the reliable review of deaths in hospital. This supports Hospital Standardised Mortality Ratio (HSMR) work, safety and quality work and links to adverse events programme.

Benefits of programme
- Improved local learning from deaths.
- Improved local action to reduce HSMR; and
- Thematic national data for improvement and improved governance at local and national level in relation to mortality reviews.
