Improving Co-ordination, Improving Lives Event

This is an opportunity to learn and share best practice in integrated care co-ordination models to support people to live well in the community.

The event is for:

Health and Social Care Partnership staff, housing colleagues, partner organisations and people with lived experience to come together to learn and share best practice in integrated care co-ordination models to support people to live well in the community.

Call for abstracts for workshop sessions and poster displays:

  • We welcome contributions for workshops and/or posters presentations from health and social care staff from across all settings (including community, primary care, housing, care home and acute settings.)

Successful abstracts will demonstrate:

  • How your work demonstrates good practice in the co-ordination of care and support for people living with long term and chronic conditions, frail older people and people living with dementia.

Please use the guidance in the flyer to complete an abstract submission form. Forms should be submitted no later than Friday 10 November 2017
