New resources to support ACP
Healthcare Improvement Scotland is currently working with the Scottish Government to develop COVID-19 specific ACP templates and guidance. Please check this page regularly for the latest national resources.
The resource below is a tool to be used in conjunction with practitioner judgement, and is not designed for sole use by individuals and their families without guidance.
- Essential Anticipatory Care Planning Guidance and Template - this resource been developed to support health and social care staff with ACP conversations and includes specific information relating to COVID-19.
- Anticipatory Care Planning guidance for people with Dementia – this letter from the Chief Medical Officer circulated on 11 September details guidance on ACP for people with Dementia.
- Coronavirus and care planning conversations - Dr Paul Baughan, a GP in Forth Valley and National Clinical Lead with Healthcare Improvement Scotland, has made this short video to explain the importance of care planning conversations for those most vulnerable to coronavirus infection.
- CPR and COVID-19 (coronavirus) conversations with patients - Royal College of General Practitioners blog post in which Dr Paul Baughan discusses care planning in the context of COVID-19.
