ihub at COSLA and Improvement Service Annual Conference 2019

The ihub is hosting a Fringe Event called 'Empowering Choices in Communities' at 1.55pm on Thursday 10 October.

During the session, we will take delegates attending the conference on a virtual tour of Scotland.

Our Portfolio Lead for People-Led Care, Chris Sutton, and other ihub team members will share examples about how we work with local partners in Scotland to empower communities and local people to have greater choice in managing their own health and wellbeing.

We'll share learning from a range of transformation and improvement programmes including:

  • community-led support,
  • anticipatory care planning,
  • the early identification of frailty, and
  • new ways of working in primary care and with housing providers.

You'll also hear how this work is promoting greater choice and improved outcomes through integrated, community based care. There will also be an opportunity to tell us about your own experiences.

In addition, you can meet us at our stand in the exhibition area during the two-day event to discuss our health and social care improvement work.
