This event will bring together health and social care staff from across Scotland to share good practice and examples of integrated care co-ordination.

By attending the day, delegates will:

celebrate the success local services have achieved by improving care co-ordination in their community
learn practical approaches to improving care co-ordination, and
network with delegates to learn from their experiences of an integrated approach to care coordination.

The event is being hosted by Healthcare Improvement Scotland’s Improvement Hub (ihub). With work being showcased from Focus on Dementia, Living Well in Communities, Place, Home and Housing and the Primary Care Portfolio.
