The session will explore how to improve the involvement of carers in hospital by looking at the whole system, what planning might need to be involved and how to identify carers from the point a patient arrives at hospital (whether planned or unplanned), how to involve and consult with carers through the stay in hospital, and how to support them through those conversations and the transition to discharge should they need it.

This event is for all key staff in these processes including front line health staff and their managers such as chief nurses, discharge without delay leads, local carer leads and carer centre managers. Carers with lived experience of discharge will also be there to add their views of where improvements could be made and to remind us of the impact on carers.
