This interactive, in-person event is an opportunity to explore a Quality Improvement approach to understanding your whole system while planning improvement activity. The event will be of interest to key professionals from across health and social care and the third sector with a role in improving carer involvement in hospital discharge planning.

The day will present opportunities to hear from and be inspired by colleagues and senior leaders, learn about how to get started on improvement projects and share good practice with others both locally and nationally.
