Frailty at the Front Door
The Frailty at the Front Door Collaborative was hibernated in March 2020 when Healthcare Improvement Scotland resources were reprioritised due to COVID-19.
Frailty remains a key area of focus in Healthcare Improvement Scotlands remobilisation. A Frailty Improvement Programme is being developed to support health and social care partnerships and NHS boards improve outcomes for people with frailty.

The aim of the Frailty at the Front Door collaborative was to improve the experience and outcomes of people with frailty who present at unscheduled care.
Evidence shows the benefits of early and effective comprehensive geriatric assessment, re-ablement and intermediate care for people living with frailty. The collaborative supported NHS boards to put this evidence into practice. We supported NHS boards to improve identification and care coordination for people with frailty who present at the front door of Acute Care.
There were two phases to the collaborative. Phase 1 was delivered between December 2017 and May 2019. Phase 2 launched in September 2019 and was hibernated in March 2020 due to COVID-19.
Phase 2 of Frailty at the Front Door
Four boards were successfully recruited to take part in the second phase of the collaborative. The collaborative supported teams to improve the way frailty is co-ordinated at the front door of Acute Care. Each team includes representation from acute care and health and social care partnerships.
The four boards taking part include:
- NHS Ayrshire & Arran (University Hospital Ayr)
- NHS Greater Glasgow & Clyde (Inverclyde Royal Hospital)
- NHS Lothian (Western General Hospital)
- NHS Tayside (Ninewells Hospital)
Phase 2 of Frailty at the Front Door
During phase 1 we worked with 5 NHS boards to test potential approaches to improving care coordination for people with frailty who present to unscheduled acute care services. A Frailty at the Front Door Collaborative Impact Report was produced to evaluated the first phase of the collaborative, this was used to design phase 2.
