Join Focus on Frailty
We are accepting applications to join the Focus on Frailty programme from Monday 13 March until Friday 28 April 2023 at 17.00.
By joining the Focus on Frailty programme participating teams will gain the following benefits:

Increased capacity and capability to apply quality improvement (QI) methodology including:

access to national data and measurement experts,
-
access to national quality improvement expertise to support the testing, implementing and scaling of new ways of working,
-
information on professional development including further training and support.

Opportunities to further develop skills and experience in relation to engaging with people with lived experience.

Opportunities to learn from clinical and topic experts relating to frailty.

A platform to share your innovations and improvements with a national audience including opportunities to meet and learn from peer teams throughout Scotland.

A chance to test changes which aim to improve integrated working and staff wellbeing.
Joining criteria
Teams wishing to apply should meet four joining criteria. This will ensure they are able to improve services for people aged 65 and over who are living with frailty or at risk of frailty. They are:
Criteria 1: A project team to deliver the programme. Teams can be from individual NHS boards, health and social care partnerships and GP Practices or be an integrated team. Teams can vary in size and composition but should be multidisciplinary, have access to data and include a project coordinator and clinical or professional lead.
Criteria 2: Organisational support to participate in the programme. This includes support from the relevant executive sponsor and alignment to strategic organisational priorities.
Criteria 3: Internal quality improvement and project support and a commitment to apply quality improvement methodology.
Criteria 4: Time for team members to participate in the improvement programme and deliver an improvement project. Participating teams are also expected to attend programme events including the first learning session on 31 May 2023.

Monday 13 March 2023: Programme open to accept applications
Monday 20 March 13:00 - 14:00 and Wednesday 19 April 13:00 - 14:00: Virtual drop in sessions to ask any questions about the programme, the criteria or how to apply. Please email his.frailty@nhs.scot to join.
Friday 28 April 2023, 17:00: Programme closed to applications
Tuesday 16 May 2023: Applicants informed of outcome
Wednesday 31 May 2023: First in-person learning session at Golden Jubilee Conference Hotel
November 2023 (date TBC): Second learning session
Spring 2024 (date TBC): Third learning session
Autumn 2024 (date TBC): Fourth learning session
Participating teams would need to ensure they are available to attend the above events.
