Evaluation of a new service model - Forres Neighbourhood Care Team

This work describes the support offered by the Evidence and Evaluation for Improvement Team to Health and Social Care (HSC) Moray with the evaluation of a new nursing model piloted in the Forres area.

The report describes a new community nursing model - Forres Neighbourhood Care Team - that embeds a collaborative way of working between housing and healthcare.

The evaluation examined:

  1. Changes in emergency admissions, 28-day re-admissions and length of stay following the introduction of the Forres Neighbourhood Care Team.
  2. Before-and-after hospital admission costs of people cared for by the team

The findings indicate that the new model began to show signs of positive impact on emergency hospital admission rates.

For the people cared for by the Forres Neighbourhood Care Team, there is some evidence of reduced costs associated with hospital admissions (reduction in number of admissions and length of stay).
