Governance framework and structure diagrams
These example diagrams have been developed with partners to help them establish a clear and joint ‘vision’ for the provision of the community equipment service: one covering the ‘drivers’; and the other, the service objectives. This supports the creation of improved governance for both the review of the service, and then ongoing delivery. This can be used to assist partners to identify the key workstreams and priorities.

Partnership Agreements
This example Partnership Agreement lays out the basis for the arrangements between NHS boards and local authorities. It also allows relevant services, such as health, social care, education and housing, to establish the governance responsibilities for the overall service, including ‘lead partner’ arrangements

Reports
This report from Western Isles HSCP provides an example of a successful bid (for an additional £102,000) to mainstream funding for the community equipment service. The report illustrates the comprehensive approach the Health and Social Care Partnership took to the overall evaluation of the service (utilising the self-evaluation templates from the Good Practice Guide), and evidences the individual actions the partners collectively implemented, or plan to do so, as a more cohesive, integrated community equipment service.
