Service model
Action plans
These action plans provide an example of an overall review of the community equipment service model, as well as a shortened example specifically for a review of children’s equipment provision. These link to the ‘strategic framework’ diagrams to provide the required detail around the tasks and actions to be implemented.

Presentation
This presentation, which can be used to begin discussions with key stakeholders around a service review, highlights the opportunities and key principles.
Protocols
The purpose of the joint protocol is to ensure a robust and consistent approach to both the assessment of need and prescription of equipment, simplifying service pathways across service, agency and geographical boundaries. This example has been developed by seven Health and Social Care Partnerships and includes the standardised roles and responsibilities for the provision of a wide range of equipment in different service settings across all care groups.
Protocols - arrangements for the provision of equipment for effective discharge
This example specifically covers the protocol arrangements for the provision of equipment for effective discharge. It clarifies the multidisciplinary assessment responsibilities and service pathways.
Protocols - children’s services partnership arrangements
This example is specific to children’s services partnership arrangements. It was developed with Tayside partners and can act as a stand-alone protocol where no other joint protocol (for wider care group provision) exists.
Protocols - education equipment
As above, this describes arrangements with education partners for the effective provision of equipment within school settings.
