Is your NHS board interested in improving the coordination of frailty at the front door of acute care?

The application period for NHS boards to apply to be part of the Frailty at the Front Door collaborative is now open.

The Frailty at the Front Door Collaborative is part of the ihub Acute Care Portfolio and is one of a number of offerings that aim to address the needs of people living with frailty across their pathway of care.

The aim of part 2 of this collaborative is to improve the processes of identifying frailty and coordinating care in hospitals to deliver better experiences and outcomes for people living with frailty.

The potential benefits of participating NHS boards include:

  • a reduction in avoidable admissions,
  • a reduction in length of stay,
  • increased bed capacity,
  • improved patient flow,
  • a reduction in re-attendance rates,
  • a clearly defined and effective pathway for frailty, and
  • significant cost benefits.  

The closing date for applications is 12 noon on Friday 10 May 2019. Interviews will take place on Wednesday 29 May by teleconference. Please note there will be no alternative dates available.
