Focus on Frailty Improvement Programme showcased at NHS Scotland Event 2023

Healthcare Improvement Scotland are delighted to present the following session at the NHS Scotland Event 2023 in Glasgow on 19 June.

“Improving Identification and Management of Frailty in the Digital Age: A Person-Centred Approach"

 

Delegates attending the event will learn about our recently launched Focus on Frailty Programme which supports teams to use a blend of quality improvement and service design methods to plan and deliver integrated services for people living with frailty. This is based on the needs of the person, their carers and families.

The Focus on Frailty Programme brings together NHS boards, Health and Social Care Partnerships and GP practices to improve the experience of, and access to person centred, coordinated health and social care for people aged 65 and over who are living with or at risk of frailty.

The programme was designed based on outputs from a 90-day learning cycle which identified the key components of an integrated frailty system.

The Healthcare Improvement Scotland Team will present alongside Midlothian Health and Social Care Partnership who will showcase some of their digital work around frailty.
