Care Co-ordination implementation site selected

We are delighted to inform you that, further to a robust selection process involving people with dementia and carers and national partner organisations, the Focus on Dementia improvement portfolio has now selected the successful site for our Care Co-ordination work.

We are looking forward to working with Inverclyde Health and Social Care Partnership to take this work forward and to share and learn with other partnerships across Scotland.

Inverclyde Health and Social Care Convener, Councillor Robert Moran, said:

"Our approach to supporting people with dementia in Inverclyde has been on building capacity to enable people with dementia to live well and remain active participants within their own community. The aim is to improve access to appropriate support and intervention at every stage in their illness. Our work towards a dementia friendly Inverclyde also aims to improve awareness and knowledge across services and the wider community. 

We are delighted that Inverclyde has been chosen as a care co-ordination implementation site. It gives us the opportunity to continue to work harder at including all people with dementia in our engagement work. A particular area of focus will be that those in our community with young onset dementia are able to access age appropriate services."
