Join our new Hospital at Home Expansion Programme to help spread Hospital at Home in Scotland
The new Hospital at Home expansion programme is now open to areas in Scotland looking to start a new service or expand their current one.
Hospital at Home (H@H) can provide a safe, patient centred alternative to an acute hospital admission. It provides a better outcome for the patient, who receives treatment in the safety and comfort of their own home, as well as reducing pressure on hospital sites.
Since 2020 we have supported H@H services across Scotland to develop and expand. 21 Health and Social Care Partnerships (HSCPs) are now supported by H@H services with 11,686 people accessing H@H services in 2022/23.
We are now looking to expand this work further by inviting services to join our new Hospital at Home expansion programme. By joining services will gain access to:
- An opportunity to bid for Scottish Government funding to support H@H developments.
- Hands on quality improvement support to identify where funding can have the biggest impact on the number of patients managed by your service.
- Quality improvement support to implement changes and set-up new services.
- Analytical expertise to demonstrate the impact of your work through data and evaluation techniques.
- A platform to share your innovations and improvements with a national audience.
- Opportunities to network and learn from other H@H services across Scotland.
NHS boards and HSCP in Scotland can apply. Our programme is open to current H@H services as well as areas that are looking to start a H@H service. Find out more about the programme and how to join on our web page.
During the application process we will be hosting virtual drop-in sessions. These will give you the opportunity to ask questions about the programme and the application process.
