Nominate for Scottish Health Awards before 29 August 2019
Healthcare Improvement Scotland is sponsoring the 'Integrated Care Award' category at the Scottish Health Awards.
This award celebrates the high quality, co-ordinated health and social care that helps the people of Scotland.
Do you know a team or individual who delivers outstanding person-centred, effective health and social care, and who should be given the recognition they deserve?
The ihub, which is part of Healthcare Improvement Scotland, supports health and social care organisations across Scotland to redesign and continuously improve with the aim of improving the health and wellbeing outcomes for everyone in Scotland.
To do this, we work with health and social care partnerships, local authorities, NHS boards, third and independent care sector organisations, and housing organisations.
Therefore our Director of Improvement, Ruth Glassborow, thinks you might know a healing hero and encourages you to send in nominations by saying:
"We are delighted to support the Integrated Care Award at the Scottish Health Awards.
"We know there is loads of fantastic work taking place throughout Scotland to design and deliver services which are truly integrated around the needs of individuals in Scotland and this is a great opportunity to raise its profile.

The Scottish Health Awards celebrate the extraordinary work of the people on the frontline of health and social care provided by NHSScotland and its partners.
Nominations are open until 29 August 2019.
