Since July 2014, NHS Fife, NHS Grampian, NHS Greater Glasgow and Clyde and NHS Highland have been taking part in a collaborative aimed to enhance communications between GPs and pharmacists working in primary care. This work has been funded from the Health Foundation’s Closing the Gap in Patient Safety Programme.

Now in its second year the collaborative has been developing and implementing a range tools to:

  • improve process reliability for the safer prescribing, monitoring and dispensing of high risk medicines, such as warfarin and non-steroidal anti-inflammatory drugs (NSAIDs)
  • improve the reliability of medicines reconciliation when patients are discharged from an acute hospital, and
  • raise awareness of the factors that contribute to ‘safety culture’ through the use of a safety climate survey.

To explain all the work undertaken during the collaboration a motion graphic has been produced.

The pharmacy teams who have taken part in the collaboration are continuing to improve the prescribing, monitoring and dispensing of high risk medicines and medicines reconciliation.

The team from NHS Fife recognised that there is a lot of information for patients taking Warfarin which can be confusing. To assist patients they created a short film which gives guidance and answers common question that patients may have. To view the film click here:

The Scottish Government recently announced plans to formalise and embed continuous improvement and safety processes in community pharmacy services, as a result the pharmacy safety climate survey is now available nationally.
