We would encourage all practice teams to share their improvement journey to help spread their learning to other teams. You can share your learning by:

• writing a learning summary of your project using this learning summary template
• share your experience with your local improvement team
• share your work and what you learned on Twitter by tagging @ihubscot and @SPSP_PC, or
• contact us by emailing us at his.pcpteam@nhs.scot to share your improvement journey with the ihub.
