Rapid Response - Admission avoidance hospital at home for older people with frailty

The guiding principles for H@H included a review of available evidence on H@H and was published in January 2020. Since its publication there have been a number of significant studies conducted in relation to H@H and therefore an updated evidence review has been produced to summarise the key findings from the most recent trials.

Wider Hospital at Home community

The Hospital at Home UK Society provides resources from across the UK for: NHS practitioners and managers, policy makers, patients and caregivers, and the academic community. The first Hospital at Home UK Society event was jointly hosted by Living Well in Communities, highlights from the day can be found in the event flash report.

The World Hospital at Home Community provides a forum for Hospital at Home services around the world to share their experiences and learning.

Data and digital

The Digital Health and Care Institute (DHI) are working with NHS Lanarkshire Hospital at Home service to use a design-led approach to identify digital technology that can help meet future need. 

NHS National Services Scotland’s Information Services Division provide a definition of Hospital at Home and are supporting the collection and analysis of data supported by their Discovery platform

Strategic planning and service redesign

Healthcare Improvement Scotland’s Strategic planning good practice framework supports Integration Authorities to develop their approach to strategic planning and commissioning. This framework could be applied to the development of local Hospital at Home services.
