Inverclyde care co-ordination

The Focus on Dementia team worked collaboratively with Inverclyde Health and Social Care Partnership, from June 2019 until March 2022. This programme of work supported improvements and redesign of community based services to improve the experience, safety and co-ordination of care for people with dementia from diagnosis to end of life care. The emphasis was on supporting people to stay well at home or in a homely setting for as long as possible.

Improving care co-ordination for people with dementia in Inverclyde report

The report was produced following an independent external evaluation of the work conducted by RSM UK Consulting LLP on behalf of the Scottish Government.

Healthcare Improvement Scotland also produced an overview report outlining the key findings and our role in the programme.

Involving and supporting unpaid carers of people with dementia 

We are pleased to share two impact stories, produced by and in collaboration with the Unpaid Carers Programme at Healthcare Improvement Scotland, on involving and supporting carers of people accessing dementia services.

Inverclyde dementia care co-ordination final webinar 

We were delighted to host 55 professionals at our final webinar on Wednesday 30 March 2022 to mark the end of the Inverclyde Dementia Care Co-ordination programme.

Dementia and palliative care identification tools guide

The Focus on Dementia team have designed a palliative care identification tools guide for staff supporting people living with dementia. The guide will make it easier to choose a tool to use or to find out more about the choice of tools available. 

Inverclyde dementia support and services leaflet

The leaflet contains information and contact details for health, social care and community services and supports in Inverclyde. This information is intended to support people living with dementia to manage their condition, to live well and independently at home and stay connected to their local community.

The leaflet was co-produced with our Programme stakeholders and the Inverclyde Dementia Reference Group. The Dementia Reference Group is for people living with dementia and their carers to share their stories, ideas, and experiences. 

Learning and innovations in Inverclyde during the first wave of the COVID-19 pandemic

Inverclyde Health and Social Care Partnership produced a case study sharing their experiences during the COVID-19 pandemic. Although not related to the Dementia Care Co-ordination Programme, it captures learning and innovations within Inverclyde HSCP that many other areas across Scotland may find of interest.
